In the diabetic eye, one of (if not the) earliest indication of pathology is an increase in the permeability of the blood retinal barrier. This barrier is normally not very permeable to bulk fluid exchange. In the early stages of diabetic retinopathy, some normally tight junctions in the eye are broken, and fluid is able to leak into the eye.
